You need to add some parameters to the template below:

project: the name of your project (e.g. for Wikibooks:WikiProject Languages, set project = Languages)
topic: the topic sort for this category (e.g. if your quality categories are Category:B-quality language pages, etc. set topic = language)
task_force: set to yes if this is an assessment category for a task force
tf_link: set to the portion of the page name following the project name (e.g. for Wikibooks:WikiProject Languages/French language task force, set tf_link = French language task force)
