How to fill in this form:
  1. Replace CURRENT with your current username.
  2. Replace NEW with the new username that you would like to be renamed to.
  3. Enter your reason for renaming by replacing Requested reason for renaming in the |3 = Requested reason for renaming line with your reason for requesting a rename.
  4. Please provide a diff confirming you own the account at the home wiki listed on the SUL tool, if that account has been registered on any WMF wiki. We want to make sure that we are not creating SUL conflicts when doing renames.
  5. Do not add a section heading; it will be automatically added.
  6. Ensure that you have followed the instructions above correctly.
  7. Click "Save page" and then watch for a reply.

  • Requested name: Mellen deadrock (SUL conflicts?) (email?) (rename user)
  • Reason: I started this account on wikibooks and made several edits before finding something I wanted to contribute to on wikipedia. I tried to create the same username on wikipedia to allow for SUL, and saw this username was already taken. So instead I have created a new username on wikipedia - not in any other wiki projects - and now want to change my wikibooks name to my wikipedia name. Reasons being to retain my contributions, make it easier to have a single login and finally so I'm not in the way of the original Ceres if they decide to use wikibooks.

Please make an edit to your talk page at Wikipedia stating "I am Ceres on Wikibooks and request the account there to be renamed to this account", while logged in at Wikipedia as Mellen_deadrock. This proves that you and Mellen_deadrock are one in the same and ensures that we're not inadvertently setting up the same situation you encountered in being unable to register Ceres.
