Microsoft Office AddIn Development Cookbook =Command Bar &Command bar in word 2003 is stored in .dot templates and .doc documents. So, actually there is no concept of application command bar. How many command bars you see, and where are they, depends on which the current document and its attached templates. It sounds quite straightforward, but not all people doing add-in development knows or remembers this fact.

According to the above information. To change/add a command bar, you either change existing document/template or you attach new template which already has the customization you want. The first approach could be done by changing or adding CommandBar object. The second approach could be done by add new add-in (which is the terminology word 2003 use to describe global template) to application add-ins.
